When does my employer coverage actually end? after 10 years as a dental hygienist

I am not sure what date my benefits stop after being let go as a dental hygienist. Thank you in advance. The letter is unclear about dental and vision.

Ask HR to confirm the exact date in writing. I also called the insurance company to double-check.
