When does my employer coverage actually end? - 5 months and counting

I am not sure what date my benefits stop after being let go as a medical assistant. I worked as a medical assistant before this. I have an appointment next week. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
