When does my employer coverage actually end? - 14 months and counting

I am not sure what date my benefits stop after being let go as a paralegal. This is my first time dealing with any of this. The letter is unclear about dental and vision. What should I do about appointments?

Keep a copy of any coverage confirmation in case of later disputes.

I moved my appointments earlier while coverage was active. It saved me a bill.
