Severance agreement arrived, need a second opinion - any advice?

I was offered 5 weeks of severance after 5 years as a store manager, and I have a few days to sign. I worked as a store manager before this. I do not want to leave money on the table.

An hour with an employment lawyer cost me a small fee and made me feel much safer about signing.
