Checking my last day of health coverage (Cincinnati)

HR said my health plan continues for a bit and I do not understand how. I worked as a warehouse supervisor before this. I have an appointment next week. How did you confirm your end date?

Dental and vision sometimes end on a different date from medical. Mine did.
