Checking my last day of health coverage after 9 years as a machine operator

HR said my health plan continues for a bit and I do not understand how. I do not usually post, so bear with me. I have an appointment next week. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.
