Checking my last day of health coverage after 4 years as a nurse's aide

I am not sure what date my benefits stop after being let go as a nurse's aide. I worked as a nurse's aide before this. The letter is unclear about dental and vision.

I moved my appointments earlier while coverage was active. It saved me a bill.
