Checking my last day of health coverage

I am not sure what date my benefits stop after being let go as a janitorial supervisor. Any experience at all would help. I have an appointment next week. What should I do about appointments?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
