Checking my last day of health coverage

I am not sure what date my benefits stop after being let go as a delivery driver. I have read a lot of threads here and could not find my exact situation. I have an appointment next week. Did coverage match what HR said?

Thanks for writing this out. I went through something close to it 4 months ago, and it settled down once I kept a simple log of every call and form.
