Checking my last day of health coverage - 6 months and counting

I am not sure what date my benefits stop after being let go as a welding apprentice. I do not usually post, so bear with me. The letter is unclear about dental and vision. How did you confirm your end date?

Thanks for writing this out. I went through something close to it 6 months ago, and it settled down once I kept a simple log of every call and form.
