Checking my last day of health coverage - 12 months and counting

HR said my health plan continues for a bit and I do not understand how. I do not usually post, so bear with me. I have an appointment next week. What should I do about appointments?

Ask HR to confirm the exact date in writing. I also called the insurance company to double-check.
