Benefits end date after a layoff - what worked for you?

I am not sure what date my benefits stop after being let go as a customer-service rep. I am in Phoenix, in case local advice matters. I do not want to be surprised by a bill.

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
