Benefits end date after a layoff (Tucson)

I am not sure what date my benefits stop after being let go as a janitorial supervisor. I would rather hear the honest version than the polite one. I do not want to be surprised by a bill. What should I do about appointments?

Thanks for writing this out. I went through something close to it 10 months ago, and it settled down once I kept a simple log of every call and form.
