Benefits end date after a layoff, payroll clerk here

I am not sure what date my benefits stop after being let go as a payroll clerk. I do not usually post, so bear with me. The letter is unclear about dental and vision. What should I do about appointments?

Keep a copy of any coverage confirmation in case of later disputes.

Ask HR to confirm the exact date in writing. I also called the insurance company to double-check.
