Benefits end date after a layoff (Nashville)

HR said my health plan continues for a bit and I do not understand how. I have been trying to stay organised about it. The letter is unclear about dental and vision. How did you confirm your end date?

Dental and vision sometimes end on a different date from medical. Mine did.
