Benefits end date after a layoff after 3 years as a procurement assistant

I am not sure what date my benefits stop after being let go as a procurement assistant. I would rather hear the honest version than the polite one. The letter is unclear about dental and vision. Did coverage match what HR said?

I moved my appointments earlier while coverage was active. It saved me a bill.
