Benefits end date after a layoff - 9 months and counting

I am not sure what date my benefits stop after being let go as a forklift operator. Thank you in advance. I do not want to be surprised by a bill. How did you confirm your end date?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
