Benefits end date after a layoff - 5 weeks in

I am not sure what date my benefits stop after being let go as an IT help-desk technician. I have an appointment next week. What should I do about appointments?

Thanks for writing this out. I went through something close to it last year, and it got easier once I kept a simple log of every call and form.
