Benefits end date after a layoff - 14 months and counting

I am not sure what date my benefits stop after being let go as a retail manager. I would rather hear the honest version than the polite one. The letter is unclear about dental and vision. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.
