Best tools for tracking applications? (update)

I am tracking my applications in a notebook and want something better. I would rather hear the honest version than the polite one. A spreadsheet with dates and follow-ups worked a little. Are there tools to avoid?

Thanks for writing this out. I went through something close to it last year, and it got easier once I kept a simple log of every call and form.
