Best tools for tracking applications? (update)

I am looking for free tools that do not spam me. I have been trying to stay organised about it. I do not want to pay for premium features. Any good templates?

Thanks for writing this out. I went through something close to it 7 months ago, and it got easier once I kept a simple log of every call and form.
