Best tools for tracking applications? (Minneapolis)

I am tracking my applications in a notebook and want something better. It has been weighing on me all week. I want reminders to follow up. Any good templates?

Thanks for writing this out. I went through something close to it 2 months ago, and it settled down once I kept a simple log of every call and form.
