Best tools for tracking applications? - 14 months and counting

I am tracking my applications in a notebook and want something better. This is my first time dealing with any of this. I want reminders to follow up. Are there tools to avoid?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
