Returning to work after caring for family (Buffalo)

I stepped out for 4 months to care for a family member and now want to return. It has been a long 22 weeks. I am unsure how to describe the break. What helped?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
