Returning to work after caring for family after 12 years as a software tester

I stepped out for 6 months to care for a family member and now want to return. This is my first time dealing with any of this. I am unsure how to describe the break. How did you get back in?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
