Returning to work after caring for family - 3 months and counting

I stepped out for 3 months to care for a family member and now want to return. There are 5 people counting on me, so I am trying to be careful. I am unsure how to describe the break. What helped?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
