Going back after 5 months at home (update)

I used to work as a machine operator and I am looking for a part-time role to start. There are 16 people counting on me, so I am trying to be careful. I am unsure how to describe the break.

Thanks for writing this out. I went through something close to it 5 months ago, and it got easier once I kept a simple log of every call and form.
