Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 13 jobs a week as a machine operator, but I am unsure what my state wants in the log. My state says to keep records for a year. Has anyone been audited?

Keep everything in writing and save the confirmation numbers. It saved me later.
