Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 12 jobs a week as a home health aide, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. I also did a résumé workshop at the library. Has anyone been audited?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.

Keep everything in writing and save the confirmation numbers. It saved me later.
