Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 14 jobs a week as a forklift operator, but I am unsure what my state wants in the log. My state says to keep records for a year.

Thanks for writing this out. I went through something close to it 3 months ago, and it settled down once I kept a simple log of every call and form.

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
