Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 5 jobs a week as an HR coordinator, but I am unsure what my state wants in the log. There are 5 people counting on me, so I am trying to be careful. I also did a résumé workshop at the library. What do you record?
