Work-search log: how detailed does it need to be? - what worked for you?

I am keeping a spreadsheet of applications, but I worry it is not what the agency expects. Any experience at all would help. My state says to keep records for a year. What do you record?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.

Screenshots of the submitted application are handy backup when a posting disappears.
