Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 12 jobs a week as a janitorial supervisor, but I am unsure what my state wants in the log. I have been trying to stay organised about it. I also did a résumé workshop at the library. Do interviews and workshops count?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
