Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 18 jobs a week as a janitorial supervisor, but I am unsure what my state wants in the log. There are 18 people counting on me, so I am trying to be careful. I also did a résumé workshop at the library. What do you record?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
