Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 11 jobs a week as a web developer, but I am unsure what my state wants in the log. I would rather hear the honest version than the polite one. My state says to keep records for a year. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.

Thanks for writing this out. I went through something close to it last year, and it got easier once I kept a simple log of every call and form.
