Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 5 jobs a week as a warehouse supervisor, but I am unsure what my state wants in the log. I worked as a warehouse supervisor before this. My state says to keep records for a year.

Screenshots of the submitted application are handy backup when a posting disappears.
