Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 4 jobs a week as a recruiter, but I am unsure what my state wants in the log. I am in Louisville, in case local advice matters. I also did a résumé workshop at the library. What do you record?

Thanks for writing this out. I went through something close to it 11 months ago, and it settled down once I kept a simple log of every call and form.

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
