Work-search log: how detailed does it need to be? - what worked for you?

I have been applying to about 14 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. I worked as a nurse's aide before this. I also did a résumé workshop at the library. Do interviews and workshops count?

Thanks for writing this out. I went through something close to it 7 months ago, and it got easier once I kept a simple log of every call and form.
