Work-search log: how detailed does it need to be? (update)

I have been applying to about 5 jobs a week as a recruiter, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. My state says to keep records for a year. Has anyone been audited?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
