Work-search log: how detailed does it need to be? (update)

I have been applying to about 12 jobs a week as a school bus driver, but I am unsure what my state wants in the log. I would rather hear the honest version than the polite one. My state says to keep records for a year. What do you record?

Thanks for writing this out. I went through something close to it 4 months ago, and it got easier once I kept a simple log of every call and form.
