Work-search log: how detailed does it need to be? (update)

I have been applying to about 14 jobs a week as a line cook, but I am unsure what my state wants in the log. I worked as a line cook before this. My state says to keep records for a year. What do you record?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
