Work-search log: how detailed does it need to be?, software tester here

I have been applying to about 9 jobs a week as a software tester, but I am unsure what my state wants in the log. This is my first time dealing with any of this. My state says to keep records for a year. What do you record?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
