Work-search log: how detailed does it need to be? (Portland)

I have been applying to about 6 jobs a week as a customer-service rep, but I am unsure what my state wants in the log. This is my first time dealing with any of this. I also did a résumé workshop at the library. Has anyone been audited?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
