Work-search log: how detailed does it need to be?, nurse's aide here

I have been applying to about 18 jobs a week as a nurse's aide, but I am unsure what my state wants in the log. I worked as a nurse's aide before this. My state says to keep records for a year. Has anyone been audited?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
