Work-search log: how detailed does it need to be?, medical assistant here

I have been applying to about 17 jobs a week as a medical assistant, but I am unsure what my state wants in the log. I also did a résumé workshop at the library. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
