Work-search log: how detailed does it need to be? (Knoxville)

I have been applying to about 15 jobs a week as a medical assistant, but I am unsure what my state wants in the log. Any experience at all would help. My state says to keep records for a year. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
