Work-search log: how detailed does it need to be? - first time posting

I have been applying to about 5 jobs a week as a warehouse supervisor, but I am unsure what my state wants in the log. I am in Providence, in case local advice matters. I also did a résumé workshop at the library. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
