Work-search log: how detailed does it need to be? (Detroit)

I have been applying to about 15 jobs a week as a medical assistant, but I am unsure what my state wants in the log. I have read a lot of threads here and could not find my exact situation. My state says to keep records for a year. What do you record?

I was asked for my log once, after six months. Having it in a spreadsheet made it painless.
