Work-search log: how detailed does it need to be? - any advice?

I have been applying to about 3 jobs a week as an accountant, but I am unsure what my state wants in the log. It has been a long 15 weeks. Some days I only network by email or attend a job fair. Has anyone been audited?

I record the date, the company, the job title, how I applied and any contact name. It takes a minute each and it covered me when I was asked for proof.
