Work-search log: how detailed does it need to be? - any advice?

I have been applying to about 10 jobs a week as a payroll clerk, but I am unsure what my state wants in the log. I worked as a payroll clerk before this. My state says to keep records for a year. What do you record?

Screenshots of the submitted application are handy backup when a posting disappears.
